Patient history taking

Date of confession: First clinical week after CRP.
Condition: Disastrous

Chief Complaint: Patient, Tan Chu Yee,21 years old, Chinese girl, will sooner or later, that is tomorrow, admitted to home sweet home  due to chief complaint of persistent lethargy and extremely low concentration.

Patient was previously well, until one week prior admission, She complaint of lethargy. It all starting the week after happy CRP trip. Patient claimed that, she felt sleepy all the time and tired.However, there is no fever, no cough and no runny nose. She has regular sleeping time which is from 1.30am until 8.00am, 7 hours, still she felt lethargy.She has currently decrease in appetite, no LOSS, but decrease. She used to take 2 bowls rice beforehand and now only half of the plate enough to bloat her stomach. She neither decrease nor increase in weight.

Besides, she also complaint of low concentration in class. She used to actively participate in class before CRP and now she feel hardly to concentrate. She is now only able to concentrate for 30 minutes before she starts to doze off. Patient also claimed that her thinking process become slow and she cant process a phrase within seconds.

One day prior admission, she complaint of headache. It was sudden onset, throbbing in nature,graded as 5/10.It lasted for 5 hours until now when she is typing.

Patient plans to get back home as soon as possible. A home trip will be good.


Differential diagnosis:
-Lazy Intrusion Syndrome
-Holiday withdrawal Syndrome
-Homesick

Investigation
- Check for GCS status
-Blood glucose level
-Blood culture for LAZY bugs

Management
-check for conciousness all the time.
-Antibiotics for lazy bugs.
-More holidays if possible.

Reminder: Patient needs to undergo End of Posting test 2 weeks later and she not yet flip her book.
